ASSESMENT SERVICES
Assessment services include all the following:
- Determining client eligibility;
- Conducting either a comprehensive assessment of clinical treatment and recovery support services needs,or recovery support assessment only,utilizing tools approved by ADP;
- Identifing the level and type of treatment and/or recovery support services needed by the client and the eligible service providers that match those needs;
- Providing an unbiased explaination of the service options to the client to ensure that they can make an informed,independent choice about the service provider(s) that will best meet their needs and personal preferences;
- Assigning a care coordinator to the client; and collecting from the client and reporting client data.
